Participant Liability Release and Waiver Form -to be completed by all athletes/alternates and adults entering the restricted areas.  All original forms need to be received in the USA office on or before January 13, 2023.  Forms must be originals.  We will not accept faxes or emails.

2023 USA Collegiate Championships Eligibility Form – form needs to be signed by Dean of Students or Registrar’s Office.  Send to the USA office on or before January 13, 2023.
